Quality Improvement Measurement and Evaluation Specialist Long Term Care (LTC) Quality Improvement (QI) Initiative
Reporting to the Quality Improvement Director, the QI Measurement and Evaluation Specialist contributes to achieving the objectives the LTC QI Initiative, “Residents First”.
The QI Measurement and Evaluation Specialist will support Residents First by developing measurement plans for each component of the initiative, and for the initiative as a whole.
The position is full time temporary (until October 30th, 2013), with the possibility of extension. The position is located in Toronto, and travel within Ontario is required.
Education and Experience:
Master’s degree in a health discipline required, or in a discipline relevant to quality improvement; a PhD is preferred.
Experience in:
· Performance measurement and applied research, working with various types of data and related analysis to support quality improvement;
· Working with a variety of health system stakeholders (required);
· Quality improvement activities or projects (desirable).

Director, Strategic Projects
Reporting to the Chief Executive Officer (CEO), and in collaboration with the senior management team, the Director, Strategic Projects is responsible for ensuring that strategic, business and project plans are developed, priorities are established, resources are secured and allocated, and the infrastructure to these projects as assigned by the Chief Executive Officer
While the CEO’s direct reports are accountable to the CEO for implementing OHQC’s vision and direction for measuring, publicly reporting and supporting improvement in the health system (as established by the Board of Directors and CEO), they also maintain an indirect reporting relationship to the COO with respect to business and project planning and control, resourcing, performance reporting, organization development and governance processes for OHQC as a whole.
The position is full-time, located in Toronto. Some travel within Ontario is required.
Education:
· MBA or equivalent.
· Certification in Project Management is an asset.
· Certification in quality improvement skills is an asset.
· Minimum of 10 years of related work experience
1. Project/Program Management
· Ability to manage multiple simultaneous activities to a common outcome, and strong knowledge of project management tools and concepts
2. Leadership Skills
· Coaching and mentorship style that fits with a corporate-wide leadership style that helps staff develop their potential, asks probing questions to help staff sharpen their problem-solving skills or create a shared vision for the work of a team
· Can effectively shape a shared vision of excellence in executing and delivering on high-profile strategic initiatives
3. Strategic Relationship Management
· Ability to develop deep connections and garner trust with stakeholders, particularly those senior Ministry of Health and Long-Term Care officials which liaise with the Council and external stakeholders with whom the Council partners on specific projects
4. Impact and Influence
· Ability to make a convincing case for OHQC’s activities and identify the potential improved outcomes for quality and cost savings to the system. The successful candidate is politically astute and has demonstrated an ability to tune in to the concerns of funders or sponsors and address them in a timely manner.
5. Passionate about quality improvement
· Can apply quality improvement tools (e.g. LEAN, process standardization, poka-yoke, templates, decision tools, 5S visual control systems, root cause analysis, PDSA cycles) to internal processes and corporate support functions to make them error-proof and efficient.

Improvement Facilitator (IF) Residents First: Residents First in Ontario Long Term Care Homes
North West Local Health Integration Network Area (NW LHIN)
Reporting to the Quality Improvement (QI) Consultant of the Residents First initiative, the Improvement Facilitator contributes to achieving the objectives of the initiative.
The Improvement Facilitator (IF) will support Residents First by working with front line teams that are participating in structured QI events, such as Collaboratives. The IF will serve as a process catalyst to support the efforts of teams and help them progress effectively to achieve their aims. The purpose of this new role is to build QI capacity within emerging Improvement Facilitators and LTC teams, supporting designated teams with QI activities, and facilitating the sustained improvements within their site.
The position is part time temporary position (0.5 FTE for 12 months, with the possibility of extension) and is located in the NW LHIN. Secondment from another organization will be considered. Some travel within the NW LHIN is required.
Education and experience:
· Undergraduate Degree in health discipline, or in a discipline relevant to quality improvement.
· Experience as a participant or leader on multiple QI teams which have used formal quality improvement tools (e.g. model for improvement, process mapping, Ishikawa diagrams, documented PDSA cycles, run charts).
· Quality improvement training

Improvement Facilitator (IF) Lead – LTC QI Initiative
Reporting to the Long Term Care Quality Improvement (LTC QI) Consultant of the Long Term Care (LTC) Quality Improvement (QI) Initiative, the Improvement Facilitator (IF) Lead contributes to achieving the objectives of the initiative by building QI capacity within emerging Improvement Facilitators and front line teams from participating long term care homes across Ontario, and facilitating sustained improvements within these LTC homes.
The Improvement Facilitator (IF) Lead is one of 6 – 8 IF Leads for this initiative. The IF lead will be a liaison between the LHIN, the provincial initiative and the local improvement facilitator in the LTC home. IF Leads develop quality improvement curricula, including LEAN, and provide coordination and mentorship of Improvement Facilitators. IF Leads also work with front line teams from LTC homes that are participating in structured QI events, such as collaboratives or LEAN Events, and serve as process catalysts to support the efforts of teams to help them progress effectively to achieve their aims. Specific assignments and responsibilities for curriculum topics and mentorship of Improvement Facilitators and front line teams will be determined by the Director of Quality Improvement, in consultation with the LTC QI Consultant and IF Leads, and may be subject to revision as the initiative progresses.
Each position is a full-time contract for the duration of the project, although part time will be considered. The term of the contract is 18 months. Secondment from another organization will be considered. Some travel within Ontario required, with most of it being within the LHIN geography that you are selected for.
