This website provides information on the quality of home care services
in 14 regions of Ontario. Each region has a community care access
centre (CCAC) that coordinates home care services for individuals. After
an individual contacts a local CCAC, a case manager assesses his or her
need for home care support and lays out all of the available care
options. Case managers work with service providers to develop a
customized care plan and to ensure that clients have access to all the
care options they need, including nursing care, Meals on Wheels,
physiotherapy, social work and nutritional counselling.
CCACs also help individuals manage their transition
from one care setting to another. They coordinate services after
hospital discharge and arrange placement into a long-term care home when
home care can no longer meet a client’s needs. By providing
services that help keep people healthy and safe at home, CCACs help
avoid unnecessary hospitalizations and readmissions, and premature
admission to a long-term care home.
The main
purpose of making information on home care quality available to the
public is to advance the quality of care and services provided to home

When searching for
information, keep in mind that we are only able to report on long-stay
clients at this time. Most of the data reported on this site come from a
new data collection tool, the RAI-HC (Resident Assessment Instrument
– Home Care). This standardized tool is now used in all CCACs,
but only for long-stay clients. Forty-six percent of home care service
recipients are long-stay clients of all ages with chronic conditions or
complex medical needs requiring care over a long period of time. The
remaining 54% are short-stay clients who require services for a short
period of time while they recover from surgery or an injury. In the
