the information available for your long-term care home, and then discuss
the results at residents’ or family council meetings. Ask
your home’s staff questions, such as:
- How frequently are problems like falls or pressure ulcers
occurring in our home?
- What is being done to reduce these problems?
- There are many good ideas for preventing these from happening.
Are we trying these ideas? If not, what are the barriers, and how can
they be addressed?
- How do we compare to other homes?
- Can we offer our own ideas or
suggestions?

reporting, ask the home’s administrator when the home might be
ready to have its information posted on this website.
